The ultimate goal of Physical Therapy is to restore maximal functional independence to each individual client. To achieve this goal, physical modalities such as exercise, heat, cold, massage, and mild electrical stimulation are utilized. Physical Therapy is provided by Physical Therapists, who are licensed health care professionals. Physical Therapists evaluate, diagnose, and manage the physical therapy treatment plan, customizing it to each individual’s needs.

Please wear or bring comfortable, loose fitting clothing that will allow the therapist to expose the affected regions of the body. Exercise wear such as sweats, t-shirts, and shorts are recommended.
Please plan to be in our facility for 60-90 minutes for your first appointment. Your therapist can tell you how long each follow-up appointment will be, usually 60 minutes.
Please arrive 15-20 minutes before your scheduled appointment time if you will be completing the new client paperwork in the office. For your convenience, we also have this paperwork on line if you would like to complete it ahead of time. Additionally, please bring the following items to your first visit:

As a courtesy, we will verify your coverage with your insurance carrier. Please note that verification of coverage is only a quote and not a guarantee of payment. When you call your insurance company to verify benefits, inform your carrier that our place of service is eleven (office setting). We will file directly with your insurance company for services rendered and assist you in obtaining payment from your insurance company. To assure the highest level of coverage, we encourage you to be aware of limitations and requirements that your insurance plan might have,

Should insurance payments be delayed, reduced, or denied, we will expect payment from you the patient, or responsible party. For your convenience, we accept cash, personal checks, money orders, Visa, Discover, and MasterCard.

Many insurance companies that we are not contracted with have “out of network” benefits that may cover a significant portion of charges.
